IMPORTANT REGISTRATION INSTRUCTIONS:
USE THIS REGISTRATION FORM ONLY IF YOU INTEND TO APPLY FOR A SCHOLARSHIP FOR LOW INCOME FAMILIES
You may register up to two children in one transaction for CineMagic participation. If you have more than two children participating, you will have to initiate another transaction WITH THIS FORM.
Please note that your scholarship may not cover the full cost of the program fee and some monetary payment may be required.
We will work with you to determine your scholarship level whether it be full or partial. Proof of income will need to be presented. Accepted documents for proof of income include MediCal, Healthy Families, Food Stamps, or Free/Reduced Lunch determination letters. Other than these, latest tax form 1040 or copies of last 3 months pay stubs from all income sources can be submitted.
After you register with this form, you may email scanned copies of your proof of income documents (make sure your name is on them) to Darren Patnoe at firstname.lastname@example.org, you may fax the documents to 805-233-6697, you may mail or bring the copies of the documents to the PVEF office at:

or you may bring copies of the documents to the Registration/Audition dates, Oct. 24 or 25. Office hours are limited so if you choose to bring the documents to the office, you may slip them under the door in an envelope if the office is closed.
Instead of filling out the online form, you can also fill out the scholarship application form and bring it, along with the proof of income documents, to one of the Registration/Audition dates.
